Responsibilities

Purpose: The Lead Clinical Social Worker functions within the Interdisciplinary Treatment Team to lead and provide patient care and supportive services that are directed at achievement of patient care outcomes, maximizing the patient’s involvement in his or her own care plan, improving service levels and decreasing costs.   Based on individual patient need and Social Work job role, this could include:  providing psychosocial assessments and interventions, completing comprehensive risk assessments, making referrals to community resources, supporting medical provider interventions, conducting health promotion activities, engaging in systems navigation and care coordination, providing ongoing case management, improving relationships between the medical provider and the patient, assisting in team building and, at times, assisting in the education and training of other healthcare providers. This position may require travel to different sites or work from fixed location assisting patients from other sites.

Knowledge, Skills, and Abilities: Flexible to meet the needs of the patients and providers. Demonstrative effectiveness in conveying information both in writing and verbally. High level of interpersonal skills to handle sensitive and confidential situations. Position continually requires demonstrated poise, tact, and diplomacy.

Ability to work independently while also being a strong team player.

 

Duties and Responsibilities:  

  • Serves as an expert and resource in the field, assistants with audits and assessment of documentation.
  • Coordinates and participates in the recruitment and training process.
  • Oversees the clinical supervision for Medical Social Work staff.
  • May also serve as clinical supervisor for Licensed Clinician staff seeking their Independent social work licensure (LISW).
  • May also be assigned as field instructor to OSU College of Social Work MSW students completing their internships.
  • Respond to billing and charting questions within the Social Work work group.
  • Position will assist with program development tasks in collaboration with the Social Work Manager
  • Assist in performing chart audits of the work group and review results with the team, lead physician and Social Work Manager.
  • Advise supervisor on staff performance issues and assists with presentation on performance data to employees.
  • Provide input and recommendations on interviewing, hiring, on boarding and training new staff.
  • Coordinate regular social worker meetings, learning sessions, continuing education and/or lunch and learns. May represent the program or hospital by participating in committees, conferences, and public meetings, and by providing education presentations to groups.
  • Participate in workshops, meetings, and conferences as needed with department and senior managers.
  • Complete documentation consistent with standards set by the compliance office.

  • Support community and healthcare linkage as needed for each client (e.g., appointments with outside outpatient providers, psychiatrist, clinical nurse specialist, or primary care physician for medication monitoring and prescribing)
  • Other duties assigned within scope of experience, expertise, and credentials.
  • Provide excellent clinical services.
  • Serve as the lead on a multi-disciplinary care team to coordinate care for patients who are living with HIV/AIDS
  • Develop, monitor, and evaluate individual case plans for each assigned client at intake, annually, and as needed thereafter

Qualifications

Requirements:

  • Master’s Degree in Social Work.
  • Independent Ohio license (LISW or LISW-S) to practice.
  • At least two years in independent clinical practice.

Understanding/Experience of Ryan White Part and Part B programs

USD $69,561.33 - USD $104,341.97 /Yr.
