Scope of Position
The Aortic Center is a comprehensive multidisciplinary program with numerous components including: educational conferences, multidisciplinary clinic, genetic evaluation, preventative care, lifelong surveillance, clinical trials, quality improvement, tissue banking, research and collaborative complex surgery.
The Aortic Center Clinic is one element of the Aortic Center that allows multiple disciplines to see patients in consultation and continued surveillance for aortic pathology. The clinical team is made up of physicians of several specialties, advanced practice providers, nursing staff, and schedulers. The current workflow of the clinic is centered on a multidisciplinary attending consultation for new referrals with comprehensive imaging and parallel lifelong surveillance which is conducted by an Advanced Practice Provider. The APP will continue to monitor the aortic pathology and once threshold for surgical consideration is met, the patient will return to the attending physician team for care planning.
Position Summary
The APP works with physicians and the nursing program coordinator to provide care for ACC surveillance patients for patients who are referred for Aortic Center Program services. The APP uses their clinical skills and knowledge for all aspects of patient care and may provide practice in both inpatient and outpatient settings. This resource will review pre-visit imaging to ensure appropriateness for the visit, may perform physical exams, order diagnostic tests, prescribe treatments, educate patients and may serve as a patient’s primary healthcare provider. The APP participates in the coordination of patient care activities in collaboration with the multi-disciplinary health team, identifies and evaluates current clinical practices through performance improvement, and provides education when appropriate.
