Responsibilities

Registered Nurse (RN)

As a Registered Nurse, you will provide skilled nursing care in a collaborative outpatient environment while helping patients achieve optimal health outcomes.

Key Responsibilities

  • Deliver skilled nursing services under the direction of physicians and advanced practice providers
  • Assess patient health needs and analyze clinical data
  • Develop, implement, and evaluate individualized plans of care
  • Monitor patient progress and modify care plans as appropriate
  • Collaborate with providers and interdisciplinary healthcare teams
  • Educate patients and families regarding health conditions, treatments, and preventive care
  • Maintain a safe, therapeutic, and patient-focused environment

Registered Nurse Care Coordinator (RNCC)

As a Registered Nurse Care Coordinator, you will play a critical role in supporting patients across the continuum of care, with a focus on coordination, communication, and improving health outcomes.

Key Responsibilities

  • Partner with providers and clinic teams to deliver evidence-based, patient-centered care
  • Support patients in managing care plans and achieving health goals
  • Coordinate care for high-risk and medically complex patients
  • Assist patients with chronic conditions, disabilities, and disease-specific care management needs
  • Facilitate smooth care transitions, including hospital discharge follow-up and transitions between care settings
  • Serve as a liaison among patients, families, healthcare providers, and community resources
  • Promote comprehensive, whole-person care through proactive communication and care coordination

 

Qualifications

Minimum Qualifications

Required for Both Positions

  • Associate Degree in Nursing from a CCNE or ACEN accredited nursing program
  • Current Ohio Registered Nurse license
  • Current CPR/BLS certification
  • Experience in clinic, physician office, urgent care, or related healthcare settings
  • Knowledge of triage procedures and patient care standards
  • Strong communication and patient education skills
  • Ability to maintain quality, safety, and infection control standards
  • Excellent interpersonal, organizational, and critical-thinking skills
  • Ability to maintain confidentiality and handle sensitive situations with professionalism and diplomacy

Pay Range

USD $32.85 - USD $48.55 /Hr.
